The 17-year-old was competing in the women's 100m breaststroke heats when the races came to a halt, as she had to get and fetch the cap that she had lost.
Following the third heat, which Toohey won, the TNT Sports cameras picked up the teenager's bright yellow cap in the middle of the pool.
"A swimming hat has come off!" TNT Sports co-commentator Grace Harvey shouted.
Main commentator James Parrack added: "It's Toohey's hat, look. That's a rare thing.
"She had a sandwich, I'm guessing, and the top hat has come off. These cameras don't miss anything, do they? If that had gone into the other lane, would she get disqualified?"
Harvey, a 100m breaststroke Paralympic gold medallist, replied: "I don't know about that. She's going to have to get back in and fish it out. We've seen it all: lanes coming off, swimming hats."
Before the fourth heat could get underway, Toohey dove back into the pool to retrieve her cap, which was met with a chorus of cheers from those in attendance.
"She's going back in to get it!" Harvey said. "The athletes that are in the next heat, it's really important that they sit back, keep warm and get ready because there's been a bit of a hold-up."
Despite the wardrobe malfunction, Toohey topped her heat in a time of 1:07.30.
The incident came after a bizarre pool malfunction delayed three-time Olympic gold medallist Adam Ramsay-Peaty's heat on Day 1 during the men’s 100-metre breaststroke.
The lane line between lanes six and seven became detached from the pool edge, which prompted a lengthy delay to ensure everything was as it should be.

"I thought I had seen everything in a swimming pool and I have never seen a lane rope come detached," Parrack said while watching the incident for TNT Sports.
Ramsay-Peaty went on to qualify for the 100m breaststroke finals, as he bids for a fifth gold medal at the Commonwealth Games.
