The move comes after an impressive season in France, where the forward netted 12 goals in 44 appearances across all competitions.
Such form helped PSG win the UEFA Champions League title, beating Arsenal in the final, while they also lifted a fifth successive Ligue 1 crown.
But new Milan head coach Ruben Amorim made Ramos a top target upon taking the reins at the San Siro.
He has now captured his man, with the 25-year-old becoming his first signing since moving to Italy.
"AC Milan today announced the signing of Goncalo Matias Ramos from Paris Saint-Germain," the club said in a statement.
"The Portuguese striker has signed a contract with the club until 30 June 2031."
Ramos is on international duty with his country, but has only featured in one of their three games at the 2026 FIFA World Cup so far.
That cap came as a late substitute as Roberto Martinez's men drew 1-1 with D.R. Congo in their opening group game.
Having qualified for the knockout stages, their next match will be in the last 32 against Croatia on Friday, July 3.
Once Portugal's involvement in the World Cup concludes, he will head to Italy as AC Milan look to improve on last season's fifth place finish in Serie A.
The Rossoneri's last league title came in 2021/22, and they have lifted only one trophy in the intervening years - the 2024 Supercoppa.

Ramos adds firepower to a squad that already includes ex-Chelsea duo Christopher Nkunku and Christian Pulisic, as well as international team-mate Rafael Leao.
"AC Milan welcomes Goncalo Ramos, and wishes him every success with the Rossoneri," the club continued in their statement.
The forward eclipses Rafael Leao - who joined from Lille for €35m (£30m) in 2019 - as Milan's record buy.
